Can I do my application over the phone? We are no longer accepting applications over the phone. Applications can be done online, or printed and mailed,faxed or emailed to us.

I completed my application but when I hit submit it returned me back to the application instead of the payment page why? If you are returned to application first check if all the *Mandatory fields are completed. This is the most common error for the application. All fields must be filled or it will return you to the page.

On the professional application and under the additional info part it asks if you are self employed, well i have my own small business and going to work for someone so what would I check? You should check "self employed"  since you own your own business. You will still be covered if you work any other place. This "box" is more for our understanding of who are members are.

I submitted my application and made a error and want to go back before I make a payment how do I do this? Once a application is submitted payment is required and you can not go back and make changes until after payment is made unless you delete your account and re-apply.

How do I delete my application? To delete your application login with your email and password then choose Yes, cancel
